The Leadership Development Report is due in the 4-H office on November 1, 2016. 

The Leadership Development Report is a new form that was created, designed, and approved by 4-H youth so that members did not have to fill out a complete form for each leadership role that they are in. If you are using the old Junior and Teen Leadership Development Report, delete it, and use the new report that is attached at the bottom of this post. All record books are required to use the new report. Why would you need to submit this report? In order to count a “Significant Leadership Role” in Section 4. Leadership Development on the Personal Development Report, the Leadership Development Report must be submitted with the Record Book. You must complete two Significant Leadership Roles to earn the Gold Star and two to earn the Platinum Star. 

There are four pages in the Leadership Development Report: Part 1 (Pre); Part 1 (Post); Part 2 (Pre); and Part 2 (Post). Part 1 of this form is dedicated to broad, general goals that members want to work on that are not specific to one project/role. Part 2 is specific to each leadership role. Part 1 (Pre) and Part 2 (Pre) are to be filled out at the beginning of the year. Part 1 (Post) and Part 2(Post) are filled out at the end of the 4-H year. Part 1 (Pre) and (Post) are only filled out one time. Part 2 (Pre) and (Post) are filled out as many times as you have leadership roles. 

For example, if a member is a club officer and a teen leader in two projects, they will fill out one Part 1 (Pre), one Part 2 (Pre) for the club officer role, one Part 2 (Pre) for the first teen leader project, and one Part 2 (Pre) for the second teen leader project. At the end of the 4-H year, they will fill out one Part 1 (Post), one Part 2 (Post) for the club officer role, one Part 2 (Post) for the first teen leader project, and one Part 2 (Post) for the second teen leader project. 

Here is a more detailed description of how to fill out the report. 

 

Part 1 (Pre) 

This page is completed at the beginning of the year. Only one page is filled out no matter how many leadership roles a member is in. Remember, Part 1 (Pre) is not specific to one project or role. These are broad overall descriptions and goals.

Fill out your name, club and the date of the program year. Put a check next to each leadership role that you hold. In the example below, the member is in the Leadership Development Project, is a Teen Leader, and is a club officer. Mark every role that applies to you!

Complete the two boxes on this page. Explain all of your leadership roles and why they are important to you.

Set personal leadership goals to be accomplished by the end of the program year. The 6 C's are six important characteristics that help young people grow into thriving and successful adults. Use these definitions of the 6 C's to help you fill out the second box. 

  1. Competence: Positive view of one's actions in specific areas, including healthy habits, life skills, love of learning, emotional competence, and social skills.
  2. Confidence: An internal sense of overall positive self-worth, persistent resourcefulness and self-efficacy.
  3. Connection: Positive bonds and relationships with people and institutions including faith-based communities.
  4. Character: Respect for societal and cultural norms, possession of standards for correct behaviors, a sense of right and wrong (morality), and integrity.
  5. Caring: A sense of sympathy and empathy for others.
  6. Contribution: Giving to self, family and the institutions of a civil society.

Part 2 (Pre) 

This page is completed at the beginning of the year. Fill out one per leadership role that you hold. In our example, Sam Green has four roles, so four individual Part 2 (Pre) pages will be filled out: one for the Leadership Development Project; one for the officer role as President; one for the Arts & Crafts Teen Leader role; and one for the Swine Teen Leader role.

This is where you record the specific goals you have for the leadership role.

Don't forget to sign Part 2 (Pre). Your leader will need to sign Part 2 (Pre). Make sure you ask your leader to do this well before the deadline! Part 1 (Pre) and Part 2 (Pre) are due to the County 4-H office on November 1, 2016. Remember to fill out one Part 1 (Pre) and as many Part 2 (Pre) as you have leadership roles. 

At the end of the 4-H year, Part 1 (Post) and Part 2 (Post) will need to be completed in order to finish the entire Leadership Development Report and submit with your record book. The Post sections are designed for you to reflect on how your experiences throughout the year contributed to your development as a leader.
